• Your provider may require a minimum of two months of treatment before completing certain documents, including but not limited to: FMLA forms, Emotional Support Animal (ESA) letters, or accommodation requests.

• Please note: We do not complete disability paperwork.

• Submission of paperwork—such as ESA letters, accommodations, or FMLA documentation—does not guarantee approval by the requesting organization or institution.

• Once your provider has completed the paperwork, our support team will fax it within 72 business hours. Please plan accordingly to meet any external deadlines.

• Providers reserve the right to decline completion of paperwork if they do not find the request clinically appropriate.
