No-Show Policy
Evolution Health Associates
The following situations are considered a no-show:
- Failing to attend your scheduled appointment
- Canceling less than 24 hours before your appointment time
- Arriving 15 or more minutes late to your session
No-Show Fees:
$50
No-show fees will be
automatically charged to the card on file on the same day the appointment is missed.
This policy applies to both
self-pay and
insurance-covered patients.
Please note: Medication refills and future appointments will not be scheduled until the no-show fee is paid in full.
