Discharge Policy

Evolution Health Associates


Patients may be discharged from Evolution Health Associates for the following reasons:

  • Disrespectful, abusive, racist, or vulgar language directed toward our support staff or clinical team.
  • Refusing to collaborate with support staff or comply with administrative processes.
  • Receiving psychiatric medications from another provider without first informing your Evolution Health Associates provider (this poses a safety risk).
  • Dishonesty or misuse of prescribed medications, including selling or misrepresenting usage.
  • Three or more missed appointments (classified as “no-shows”) without proper notice.
  • Failure to establish a payment plan for outstanding balances.
  • Refusal to keep an active credit card on file, which is necessary for our scheduling and billing operations.
  • Failure to follow up as clinically recommended by your provider.
  • Disrespectful or inappropriate behavior toward your provider, including harassment or vulgar communication.
  • Dishonesty related to legal matters, including custody situations.
  • If the patient’s needs fall outside the scope of our clinical expertise.


In the event of discharge, we will provide:

  • Three referral options to help you continue care elsewhere
  • A 60-day supply of medication, if deemed clinically safe by the provider
